What is a Patient Care Report?
A Patient Care Report (PCR) is a medical document used primarily by EMS/ambulance services to record patient information, vital signs, treatments provided, and transport details during emergency medical transportation.
When do NEMT providers need to use PCRs?
NEMT providers use PCR-like documentation in three situations: (1) higher-acuity transports involving stretcher, gurney, or Basic Life Support services where patient monitoring is required by state health departments or insurers, (2) Medicaid and insurance billing that requires trip sheets documenting medical necessity, mobility levels, and assistance provided, and (3) waiver programs like NY's NHTD/TBI that mandate care documentation for service plan compliance and incident reporting, even if not formally called a "PCR."
Who typically fills out a Patient Care Report?
A Patient Care Report (PCR) is typically completed by the primary care provider or the highest-certified individual attending to the patient, such as a Paramedic or EMT. In standard Non-Emergency Medical Transportation (NEMT), this responsibility may falls to the driver, who documents the patient’s mobility, assistance provided, and trip logistics to ensure clinical continuity, legal protection, and accurate billing.
